Welcome to Your Personal Health Record, a free Web-based service Emory Healthcare offers that allows you and your family to securely store personal health information online and gives you the ability to participate in the medical management process.

As an Emory Healthcare patient, you are our partner and your safety is our top concern. You can play an important part in helping ensure patient safety and delivery of the quality care you deserve by giving your doctor the complete picture. Carry a current list of your medications with you at all times , including name, dose, frequency and drug allergies. We encourage you to complete a Universal Medication History Form and bring it with you to all doctor's appointments and hospital visits. Your Personal Health Record is available via Cerner Corporation's IQHealth. Currently, Emory Healthcare is using only a portion of IQHealth - the portion that provides a secure tool to store your personal health information.

The information that you enter is absolutely secure and inaccessible to anyone but you (or individuals that you approve to share it with) through a function that allows you to grant access to your record to a significant other. So, if you have children, aging parents or any family member who may need assistance in accessing the system, you may set up a record for them that you can access and maintain on their behalf.

Patient benefits
You can use this secure Web site to keep track of your immunizations, allergies and any types of procedures you may have had, as well as any medications you are taking. And, you can keep the information current yourself to assure that you have all the latest available data when you need it.

For those who have special medical conditions that require consistent monitoring, the program allows you to input daily recordings of critical information. For now, the system has special sections for four conditions:

  • Diabetes: download glucometer readings
  • Congestive Heart Failure: vital sign diary
  • Asthma: peak flow meter readings
  • Sleep Disorders: sleep diary

Another feature is an A to Z health information library that can be used as a resource to learn more about a variety of medical conditions. The library provides information about health conditions or injuries, diseases, medications and even wellness guidelines.
